Reporting to you is tailored to your needs, and is provided in a practical and succinct way. This ensures your work is more efficient, and the service adds value rather than become a hindrance to your practice. Most HMR reports are only one-page, accompanied by an up-to-date medication list that includes OTC and Complimentary & Alternative Medicines (CAMs) used by the patient. Please ask us for a sample report.

Recommendations are evidence-based, and aim to support you in your clinical decisions to optimise the pharmacotherapy your patient receives.

There is a focus on chronic disease management, patient compliance and education. This improves the outcomes sought by your clinical decisions, and ensures the best cooperation by our patient.

HMRs are conducted only by Consultant Pharmacists accredited by the Australian Association of Consultant Pharmacy (AACP). Our Principal Consultant Pharmacist, Kyrillos Guirguis, also holds a Master's degree in Clinical Pharmacy and has extensive clinical and research experience. 

Your involvement in HMR is reimbursed under Medicare as MBS Item 900.

This is an MBS item for permanent residents of a residential aged care facility (RACF). It involves collaboration between a GP and a pharmacist to review the medication management needs of a resident. GP payment for the review under the MBS occurs until after the second patient consultation.
